While I agree with everyone's inclusion of Eternal Champions, I think it's forgivable when you consider the Sega CD version, which fixed nearly all of its problems.

That said, I'd like to suggest Jurassic Park for this list. It did a lot of things right, potentially a really good platformer, but the clunky jumping mechanics and physics and constant slowdown make it difficult to enjoy to its fullest.
