The only Genesis game that I've recently tried hard to beat, and had no luck, is...Shaq-Fu on Hard mode (or whatever the highest difficulty is called).
I couldn't find a reliable pattern for two of the three enemies in the second group, and with limited continues, I eventually gave up in frustration. I'll try again at some point, perhaps with a Game Genie code to allow me to repeatedly practice on certain enemies (practicing in emulation isn't a great option since I don't have a six-button controller with the right layout).
There are a few FMV games on the Sega CD that require total memorization on Hard, like Supreme Warrior and some parts of Star Wars: Rebel Assault. This was maybe the worst idea of all associated with FMV games, ever -- since without the cues, the gameplay (what little of it there is) is simply nonexistent. Not sure when I'll bother with those, if ever. In principle I'd like to finish SW:RA, but memorizing a long sequence of timings and moves in an already-obnoxiously-obscure sequence gets into FML territory.
I once might have thought that Dark Castle on Hard, Sol-Feace on Mania, etc. were out of reach -- but I beat 'em. I haven't played any of the other "hard" games on my list enough to reach that point of futility -- Chakan, Toki, Turrican -- though the avalanche sequences in Cliffhanger (both versions) looked brutal.
EDIT: Thought of one more -- Evander Holyfield's Real Deal Boxing. I won't say "never", but I'm genuinely not sure how I'm going to beat it, since it takes 30 fights to get to the top but your boxer's skills start to deteriorate after 25 fights.

Zombies Ate My Neighbors. I made a serious run at it during this last spring break and made it to level 42 before I ran out of survivors. I even had the GameFaqs page open next to me the whole time for tips and I still got owned. This game has been taunting me since '94, it is really becoming an issue for me. Also I have never beat Mega Bomberman but I WILL get that one down one day.
Surprised with all the Comix Zone comments. Maybe its because its been a while since I beat it and it was my jam as a kid, but I never had any serious problems with it.
Dark Wizard on Sega CD, though I really love the game I just don't think I have the patience for the resource hunting that each scenario requires. Last time I marathoned through Dark Wizard I ended up stuck without enough resources to continue and had to restart entirely.
comix zone, batman & robin, alien soldier. those are about the only genesis games that I haven't been able to beat. although luckily in comix zone there's a infinite health code that can be entered at the options screen to make it alittle more bearable.
Speaking of Comix Zone, I just remember that I was only able to see the end of the game by using a level select code. I may have been able to beat it, but I believe in level 4, there's a spot where you have to jump over a pit of lava, and every time I'd jump, Sketch would freeze in mid air, and I couldn't do anything except turn the game off.
Has this happened to anyone else?
I haven't even made it to level 4 yet.
The only games that I have enough interest in to play through which I might not complete are Japanese language RPGs. I got stuck in Cosmic Fantasy 1 bitd and traded away my Mega-CD before getting past that point. Someday I'll start from the beginning again and hopefully finish the entire game.
Android Assault (Level 7) = was a fun little shooter til Level 7 destroyed it.
fatal rewind is one of my favorite games but its pretty hard.
id have to go with batman and robin, that game is impossible for me, still enjoy it though

I have to agree with Android Assault, but Stage 7 did not make me quit.
I liked the challenge of that stage, and eventually adjusted my skill to its difficulty level.
