The fan on my PC runs like every minute the damn computer is on, what is going on? I check to see if its running any programs and it is only like at 4% CPU usage...what can I do? Does it need cleaning perhaps?

did it always do this? if not, it might need cleaning. I'm guessing its operating off of some sort of temperature gauge and if its excessively dusty to the point that it is insulating hot areas or covering the fan blades in a way that changes the shape and lowers effectiveness that could be your problem.
Most of the fans in my PCs run every minute the computer is on, I assume you're refering to a temperature controlled CPU fan which again will always be running, you just cant hear it under normal CPU usage.
As a safety feature, if my ASUS CPU fan does not find the temperature sensor attached (cant get a reading) then it runs at full whack all the time.
what I do is put the fans running at 5V instead of 12V... you get a bit more heat (maybe 5C more) and lot less noise.
