So I spent some time and re-tried the same mod on another console (a VA1), desoldered the original AV port and added the 1/8" headphone and it worked perfectly, so the VA3 must have something faulty....

The headphone socket is a stereo socket, but no, I'm getting mono out of it (and I double checked the pinout, it's definitely wired correctly, and there's no continuity/short between channels)
